Job summary

SummitWest Care is a nonprofit home health organization providing one-on-one occupational therapy across Mesa County, CO. You will evaluate and treat patients in their homes, focusing on safety, function, and independence with tailored treatment plans.

You’ll enjoy flexible daytime scheduling, autonomy in the field, and mileage reimbursement while being supported by an interdisciplinary team committed to exceptional patient outcomes and clinician success.

Qualifications

• Current Colorado OT license in good standing.
• Graduate of an accredited Occupational Therapy program.
• Strong clinical assessment, communication, and documentation skills.
• Ability to work independently in a home health environment.
• Excellent time-management and organizational skills.
• Valid driver’s license and reliable transportation.

Responsibilities

• Perform occupational therapy evaluations, reassessments, recertifications, and discharges
• Develop and implement individualized treatment plans focused on safety, function, and independence
• Assess patients' ability to performs ADLs
• Conduct home safety evaluations and recommend adaptive equipment or environmental modifications
• Educate patients and caregivers on home programs, mobility strategies, and safety techniques
• Monitor patient progress and adjust treatment plans as appropriate
• Collaborate with nurses, therapists, caregivers, physicians, and family
• Supervise and support COTAs when applicable
• Complete accurate and timely clinical documentation

Full-Time | Flexible Daytime Schedule

$48-55/hour DOE + Mileage Reimbursement

SummitWest Care is a local nonprofit home health agency seeking a compassionate Occupational Therapist (OT) to provide one-on-one care throughout Mesa County. Home health offers a different therapy experience.
