Job Summary

The Occupational Therapist (OT) provides skilled therapy services to patients in their homes under physician orders. Care is delivered in accordance with the plan of care, agency policies, and all regulatory standards. The OT promotes independence, safety, and functional participation in daily activities.

Essential Functions
  • Perform comprehensive occupational therapy evaluations, including OASIS assessments, within required timeframes
  • Assess functional status related to ADLs/IADLs, upper extremity function, cognition, mobility, and home safety
  • Obtain and manage physician orders for certification, recertification, and plan of care updates
  • Deliver skilled, evidence‑based occupational therapy interventions based on patient diagnosis and clinical presentation
  • Provide therapeutic activities, neuromuscular re‑education, ADL/IADL retraining, and adaptive/compensatory techniques
  • Utilize appropriate pain management strategies within scope of practice
  • Modify or discontinue interventions based on ongoing reassessment and clinical judgment
  • Identify and report changes in functional status, cognition, pain, or safety risks
  • Communicate clinically significant findings to the physician and interdisciplinary team in a timely manner
  • Evaluate effectiveness of interventions and adjust the plan of care accordingly
  • Provide education on safe performance of daily activities and use of adaptive strategies
  • Reinforce fall prevention, safety awareness, and caregiver training
  • Utilize effective communication techniques to support patient engagement and compliance
  • Identify environmental hazards and implement interventions to reduce risk of injury
  • Collaborate with physicians, nursing staff, case managers, and other disciplines
  • Supervise Occupational Therapy Assistants (OTAs) in accordance with state regulations and agency policy
  • Participate in interdisciplinary team meetings, case conferences, and care planning
  • Complete accurate, timely, and objective documentation in the EMR
  • Maintain required clinical documentation, including evaluations, visit notes, progress notes, discharge summaries, and OASIS
  • Ensure compliance with documentation, billing, and regulatory requirements
Requirements

Physical And Environmental Requirements
  • Prolonged standing, walking, bending and reaching required.
  • Ability to lift, push and move up to 50 lbs.
