Job summary

Summit Home Care and Hospice is seeking an experienced Occupational Therapist (OT) to join our patient‑centered home health team. You will evaluate functional status, assess home environments, develop treatment plans, and coordinate with physicians to deliver high‑quality, independent living support in patients' homes or local offices.

You will work a flexible schedule, travel to patients, supervise assistants as needed, and contribute to discharge planning.

Qualifications

• Graduate of an accredited occupational therapy curriculum.
• Active Ohio OT license to practice.
• 2 years of clinical OT experience preferred; home health or community-based preferred.
• Strong verbal and written communication, organization, and ability to work independently and in a team.

Responsibilities

• Evaluate patient functional status and home environment for safety and independence.
• Develop and implement treatment goals and plans; monitor progress.
• Coordinate plan of care with physicians and care team; discharge planning.
• Supervise assistants per policy and state regulations; educate family/caregivers.

We’re committed to providing comprehensive healthcare services in the comfort of our patients' homes to positively impact quality of life by redefining the delivery of care.Occupational Therapist (OT):Hours: Monday - Friday; Standard Business Hours & Flexible SchedulingAnnual Salary: $80,000 – $110,000 per YearCoverage Location: Grove City, OH & Surrounding AreasWork Setting: On the Road for Patient Visits & Training at the Local Office or RemotelyResponsibilities:Evaluate patient's functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity, etc.).Evaluate home environment for hazards or barriers to more independent living. Identifies equipment needs.Participates in the development of the total plan of care.For patients who plan to return to work, the occupational therapist may perform work capacity evaluation and may refer to specialized vocational training in accordance with Organization policy.Develop treatment programs and establish goals for improved function.Communicate the plan of care to referring physicians and other Organization professionals.May teach new skills or retrain patients in once familiar daily activities that have been lost due to illness or injury, in accordance with organization policy.May fabricate splints and instruct patients in the use of various types of adaptive equipment to improve function.May train patients in the use of prosthetic and/or orthotic devices.Maintain and submit appropriate clinical records, clinical notes, and reports to the physician about any changes in the patient's condition.Maintain contact/communication with other personnel involved in the patient's care to promote coordinated, efficient care.Attend and contribute to in-services, case conferences, and other meetings as required by company policy to ensure coordinated and comprehensive plans of care for patients.Identify patient and family/caregiver needs for other home health services and refer to them as appropriate.Instruct patient's family/caregiver and other organization health care personnel in patient's treatment regime when indicated and appropriate.Supervises certified occupational therapy assistants according to organization policy and state regulations.When therapy is the only skilled service, instruct, supervise, and evaluate home health aide care every two (2) weeks.Participates in discharge planning for patients.Qualifications:Graduate of an accredited occupational therapy curriculum.Registered with the National Registration Examination of the American Occupational Therapy Association.Active license to practice as an occupational therapist in the state of Ohio.2 years of previous clinical experience as an occupational therapist, preferred.Prior home health or community-based experience is preferred but not required.Strong verbal and written communication skills, excellent organizational abilities, and the capacity to work independently and as part of a team.Why Summit?We offer a well-rounded benefits package to support you both at work and at home.
