SummitWest Care in Grand Junction, CO, seeks a dedicated Occupational Therapist (OT) for home health. The role focuses on independent, patient-centered care with one-on-one visits and real-world goal achievement.

You will evaluate, treat, and educate patients in their homes, tailoring plans to safety and independence, while collaborating with an interdisciplinary team. Flexible daytime hours support work-life balance.

Qualifications

• Active Colorado OT license in good standing.
• Graduate of accredited Occupational Therapy program.
• Ability to work independently in home health.
• Strong communication and documentation skills.

Responsibilities

• Perform OT evaluations, reassessments, recertifications, and discharges.
• Develop and implement individualized treatment plans for safety and independence.
• Assess patients' ability to perform ADLs.
• Conduct home safety evaluations and recommend modifications.
• Educate patients and caregivers on home programs and safety.
• Monitor progress and adjust treatment plans as needed.
• Collaborate with nurses, therapists, physicians, and family.
• Supervise and support COTAs when applicable.
• Complete accurate and timely clinical documentation.

$48-55/hour DOE + Mileage Reimbursement

Responsibilities

• Assess patients' ability to performs ADLs
• Conduct home safety evaluations and recommend adaptive equipment or environmental modifications
• Educate patients and caregivers on home programs, mobility strategies, and safety techniques
• Monitor patient progress and adjust treatment plans as appropriate
• Collaborate with nurses, therapists, caregivers, physicians, and family
• Supervise and support COTAs when applicable
• Complete accurate and timely clinical documentation

Qualifications

• Current Colorado Occupational Therapist (OT) license in good standing
• Graduate of an accredited Occupational Therapy program
• Strong clinical assessment, communication, and documentation skills
• Ability to work independently in a home health environment
• Excellent time-management and organizational skills
• Valid driver's license, reliable transportation, and auto insurance

Home health experience is a bonus, but not required!
