Job summary

Summit Home Care and Hospice is seeking an experienced Occupational Therapist to join our field-based team providing home health services in Marion, OH and surrounding areas. The OT will evaluate functional status, develop plans of care, and train patients and families for safe home-based activities and work re-entry as needed.

Full-time, standard business hours with on-the-road patient visits and remote/office training opportunities.

Qualifications

• Graduate of an occupational therapy curriculum accredited by AOTA/AMA.
• Registered with the National Registration Examination of the American Occupational Therapy Association (NBCOT).
• Must be a licensed driver with an automobile in good working order and insured.
• Current CPR certification.
• Recent TB test is required.
• Two years of clinical experience as an occupational therapist.

Responsibilities

• Evaluate patient functional status (muscle function, endurance, coordination, communication, self-care, work capacity).
• Evaluate home environment for hazards or barriers to independence; identify equipment needs.
• Participate in development of the total plan of care.
• Perform work capacity evaluations and refer to vocational training when appropriate.
• Develop treatment programs and establish goals for improved function.
• Communicate plan of care to referring physicians and team members.
• Instruct and retrain patients in daily activities as needed.
• May fabricate splints and instruct on adaptive equipment use.
• Train patients in prosthetic/orthotic device use.
• Maintain and submit clinical records and reports to physicians.
• Coordinate with other care team members for comprehensive care.
• Attend in-services and case conferences as required.
• Identify needs for other home health services and refer as appropriate.
• Instruct family/caregivers and other staff on treatment regime.

Occupational Therapist (OT):

Full-Time Hours: Monday - Friday; Standard Business Hours

Annual Salary: $80,000 - $100,000 per Year

Coverage Location: Marion, OH & Surrounding Areas (Including Delaware, Richwood, Sunbury, and Cardington, OH)

Work Setting: On-the Road for Patient Visits & Training at the Local Office or Remotely

Responsibilities:

• Evaluate patient's functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity, etc.).
• Evaluate home environment for hazards or barriers to more independent living. Identifies equipment needs.
• Participates in the development of the total plan of care.
• For patients who plan to return to work, the occupational therapist may perform work capacity evaluation and may refer to specialized vocational training in accordance with Organization policy.
• Develop treatment programs and establish goals for improved function.
• Communicate the plan of care to referring physicians and other Organization professionals.
• May teach new skills or retrain patients in once familiar daily activities that have been lost due to illness or injury, in accordance with organization policy.
• May fabricate splints and instruct patients in the use of various types of adaptive equipment to improve function.
• May train patients in the use of prosthetic and/or orthotic devices.
• Maintain and submit appropriate clinical records, clinical notes, and reports to the physician about any changes in the patient's condition.
• Maintain contact/communication with other personnel involved in the patient's care to promote coordinated, efficient care.
• Attend and contribute to in-services, case conferences, and other meetings as required by company policy to ensure coordinated and comprehensive plans of care for patients.
• Identify patient and family/caregiver needs for other home health services and refer to them as appropriate.
• Instruct patient's family/caregiver and other organization health care personnel in patient's treatment regime when indicated and appropriate.
• Supervises certified occupational therapy assistants according to organization policy and state regulations.
• When therapy is the only skilled service, instruct, supervise, and evaluate home health aide care every two (2) weeks.
• Participates in discharge planning for patients.

Qualifications

• Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association.
• Registered with the National Registration Examination of the American Occupational Therapy Association.
• Must be a licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
• Possess and maintains current CPR Certification.
• Recent TB test, required.
• Two years of clinical experience as an occupational therapist.
• Demonstrates good verbal and written communication, and organizational skills.
