Job summary

Emblem Healthcare in Arizona is seeking an Occupational Therapist (OT) for home health care. You will develop individualized care plans, teach adaptive skills, and collaborate with a caring team to help patients regain independence.

The role offers a starting annual salary of $100,000 for new grads, up to $140,000 depending on experience, with full-time hours and flexible scheduling in the West Valley area.

Qualifications

• Licensed Occupational Therapist in the State of Arizona.
• Graduate of an ACOTE-accredited OT program, eligible for NBCOT certification.
• Current CPR certification.
• Valid driver’s license, insurance, and reliable transportation.

Responsibilities

• Develop and implement individualized care plans
• Provide therapy services per physician’s orders
• Assess functional status and recommend adaptive equipment
• Teach new skills or retrain patients in daily activities
• Maintain accurate documentation and participate in case conferences
