Job summary

Home Sweet Home Health in East Valley, AZ is seeking a compassionate Occupational Therapist (OT) to join our Home Health team on a Per Diem (PRN) basis. The role involves in-home evaluations, ADL-focused treatment plans, and rehab for patients recovering from illness, injury, or surgery.

You will collaborate with physicians, nurses, PTs, SLPs, and the care team; maintain EMR documentation and Medicare compliance while providing safe, patient-centered care.

Qualifications

• Current Arizona OT license.
• Graduate of an ACOTE-accredited Occupational Therapy program.
• Minimum one year OT experience; Home Health preferred.
• Strong communication, organization, and time-management skills.
• Valid driver’s license and reliable transportation.

Responsibilities

• Complete in-home OT evaluations and reassessments.
• Develop individualized treatment plans to improve independence with ADLs.
• Provide rehabilitation for patients recovering from illness, injury, surgery, or hospitalization.
• Recommend adaptive equipment, home modifications, and fall prevention strategies.
• Educate patients and caregivers on safety, mobility, and functional independence.
• Collaborate with physicians, nurses, PTs, SLPs, and the interdisciplinary care team.
• Document patient progress accurately using EMR while maintaining Medicare and agency compliance.

Occupational Therapist Responsibilities

Qualifications

• Current Arizona Occupational Therapist (OT) license


• Graduate of an ACOTE-accredited Occupational Therapy program


• Minimum one year of Occupational Therapy experience (Home Health preferred)


• Strong communication, organization, and time management skills


• Experience with EMR documentation


• Valid driver's license, reliable transportation, and current auto insurance
