Job summary

Home Sweet Home Health is seeking a compassionate Occupational Therapist (OT) to join our Home Health team in the East Valley, AZ. This per diem (PRN) opportunity offers flexible routing and a supportive interdisciplinary team to help patients regain independence at home.

As an OT with Home Sweet Home Health, you will conduct in-home evaluations, create individualized ADL-focused plans, and advocate for safety and independence while coordinating care with physicians, nurses, PT, SLP, and other

Qualifications

• Current Arizona OT license.
• Graduate of an ACOTE-accredited Occupational Therapy program.
• Minimum one year of Occupational Therapy experience (Home Health preferred).
• Strong communication, organization and time management skills.
• Experience with EMR documentation.

Responsibilities

• Complete in-home OT evaluations and reassessments.
• Develop individualized treatment plans to improve independence with ADLs.
• Provide rehabilitation for patients recovering from illness, injury, surgery, or hospitalization.
• Recommend adaptive equipment, home modifications, and fall prevention strategies.
• Educate patients and caregivers on safety, mobility, and functional independence.
• Collaborate with physicians, nurses, PTs, SLPs, and the interdisciplinary care team.
• Document patient progress accurately using EMR while maintaining Medicare and agency compliance.

Occupational Therapist Responsibilities

• Collaborate with physicians, nurses, Physical Therapists (PT), Speech-Language Pathologists (SLP), and the interdisciplinary care team


• Document patient progress accurately using EMR while maintaining Medicare and agency compliance

Qualifications

• Current Arizona Occupational Therapist (OT) license


• Graduate of an ACOTE-accredited Occupational Therapy program


• Minimum one year of Occupational Therapy experience (Home Health preferred)


• Strong communication, organization, and time management skills


• Experience with EMR documentation


• Valid driver's license, reliable transportation, and current auto insurance
