Job summary

Agape Hospice & Homecare is seeking a licensed Occupational Therapist to deliver home health OT services in Arizona. You will assess patients, design individualized treatment plans, and teach adaptive skills to promote independence, working with a caring team to support patient goals.

The role offers flexible scheduling and strong team support, with emphasis on autonomy in patient care and ongoing clinical education. Must maintain license and comply with home health regulations.

Qualifications

• Active OT license in Arizona (AZ) required.
• Graduate of ACOTE-accredited OT program; NBCOT certification eligibility.
• CPR certification required; valid driver's license and reliable transportation.

Responsibilities

• Develop and implement individualized OT care plans.
• Provide therapy services per physician's orders.
• Assess functional status and recommend adaptive equipment.
• Teach new skills or retrain patients in daily activities.
• Maintain accurate documentation and participate in case conferences.
