The Pennant Group, Inc. is hiring a Licensed Occupational Therapist to join our Colorado home health team in Cortez. You’ll develop personalized care plans, deliver therapy per physician orders, and support patients toward independence.

Qualifications

• Active Colorado OT license.
• ACOTE-accredited OT program completed.
• NBCOT certification eligibility.
• CPR certification current.
• Valid driver’s license and reliable transportation.

Responsibilities

• Develop and implement individualized care plans.
• Provide therapy services per physician orders.
• Assess functional status and recommend adaptive equipment.
• Teach new skills or retrain patients in daily activities.
• Maintain accurate documentation and participate in case conferences.
