Job summary

Transition Home Healthcare in Houston, TX is seeking an Occupational Therapist (OT) to deliver home health services, develop care plans, and support patients and families.

This full-time role requires Texas OT licensure, knowledge of home health regulations, and strong communication; driving is required to serve multiple service areas.

Qualifications

• Texas-licensed Occupational Therapist required.
• Home health experience preferred.
• Knowledge of home health regulations and general healthcare compliance.
• Able to handle physical workload.
• Good driving record.
• Experience with HCHB EMR preferred.
• Effective communication and people skills.

Responsibilities

• Facilitate rehabilitation of patients with emotional, physical, or mental disabilities.
• Develop and implement occupational therapy care plans.
• Coordinate care with home healthcare team.
• Evaluate therapy needs and monitor and report patient progress and outcomes.
• Educate patients and family members.
• Prepare timely, complete, and accurate clinical notes.
• Communicate effectively with patients, caregivers, healthcare providers, office support, and other care team members.
