Job summary

Transition Home Healthcare in Houston, TX is seeking a dedicated Occupational Therapist (OT) for home health visits across Northeast, Southeast, and Southwest service areas.

The role focuses on developing and implementing personalized OT care plans, coordinating with the home health team, educating patients and families, and documenting progress with the EMR. Strong communication and mobility are required for success.

Qualifications

• Licensed as an Occupational Therapist in Texas.
• Home health experience preferred.
• Knowledge of home health regulations and general healthcare compliance.
• Able to handle physical workload.
• Good driving record.
• Experience with HCHB EMR (preferred but not required).
• Effective communication and people skills.

Responsibilities

• Facilitate rehabilitation of patients with emotional, physical, or mental disabilities.
• Develop and implement occupational therapy care plans.
• Coordinate care with home healthcare team.
• Evaluate therapy needs and monitor and report patient progress and outcomes.
• Educate patients and family members.
• Prepare timely, complete, and accurate clinical notes.
• Communicate effectively with patients, caregivers, healthcare providers, office support, and other care team members.

Shift Available: Full Time & PRN

Service Areas for Coverage: Northeast, Southeast, Southwest

Responsibilities:
