Job summary

TheraCare is seeking a licensed Occupational Therapist (OT) for a Full-Time or PRN home health opportunity providing in-home physical therapy services to adult and geriatric patients throughout Baytown, TX and surrounding areas.

This Full-Time or PRN home health OT position offers flexibility, autonomy, and the ability to provide one-on-one patient care in the home setting, with scheduling freedom and focus on improving mobility and independence.

Qualifications

• Doctorate in Occupational Therapy from an accredited program.
• Active OT license in Texas or eligible Compact Licensure.
• Home Health experience (1 year required).
• HomeCareHomeBase (HCHB) experience preferred.
• Current CPR certification.
• Strong communication, clinical judgment, and time-management skills.
• Knowledge of home health regulations and Medicare guidelines preferred.
• Valid driver’s license, reliable transportation, and current auto insurance.

Responsibilities

• Perform home health occupational therapy evaluations and re-evaluations for adults and geriatrics.
• Develop and update individualized Plans of Care (POC).
• Treat patients with orthopedic, neurological, cardiac, post-surgical, or other medical conditions.
• Address ADLs, IADLs, coordination, strength, balance, and functional mobility.
• Apply manual therapy techniques and non-complex modalities (heat, cold, e-stim, etc.).
• Implement interventions for fall prevention, home safety, and environmental modifications.
• Train patients and caregivers in adaptive equipment and DME.
• Fabricate splints and recommend assistive devices as appropriate.
• Complete accurate and timely documentation in the EMR.
• Communicate with physicians and interdisciplinary care teams.
• Supervise COTAs and home health aides where applicable.
• Follow home health regulations and safety standards.
