Job summary

Transition Home Healthcare in Houston, TX is hiring a dedicated Occupational Therapist (OT) for full-time and PRN home health duties. You will develop care plans, coordinate with the home health team, and educate patients and families to improve outcomes.

The role requires Texas OT licensure, knowledge of home health regulations, and strong communication. Experience with HCHB EMR is a plus; valid driving record and physical stamina are essential.

Qualifications

• Occupational therapist licensed to practice in Texas.
• Home health experience highly preferred.
• Knowledge of home health regulations and general healthcare compliance.
• Able to handle physical workload.
• Good driving record.
• Experience with HCHB EMR (preferred but not required).
• Effective communication and people skills.

• Facilitate rehabilitation of patients with emotional, physical, or mental disabilities.
