Job summary

Transition Home Healthcare, an elite home health agency serving Houston, TX and surrounding areas since 2010, is seeking a Certified Occupational Therapist Assistant (COTA) to join our growing team. We offer FT and PRN shifts in the Southwest and Southeast service areas.

The COTA will implement therapy plans, coordinate care with the team, educate patients and families, and document clinically. Texas license required; driving is required.

Qualifications

• Must be a Certified Occupational Therapist Assistant (COTA) licensed in Texas.
• Home health experience is highly preferred.
• Knowledge of Home Health regulations and general healthcare compliance.
• Ability to handle physical workload and maintain a good driving record.

Responsibilities

• Facilitation of rehabilitation of patients with emotional, physical or mental disabilities.
• Implementation of occupational therapy care plans.
• Coordination of care with entire care team.
• Educating patients and family members.
• Preparing timely, complete and accurate clinical notes.
• Communicating effectively with patients, caregivers, healthcare providers, office staff and other care team members.

Responsibilities

• Home Health experience highly preferred
• Knowledge of Home Health regulations and general healthcare compliance
• Ability to handle physical workload. Good driving record.
