Job summary

Transition Home Healthcare in Houston, TX is seeking an Occupational Therapist (OT) to join our home health team. This role offers PRN and full-time shifts, serving diverse coverage areas across Houston.

The OT will develop and implement care plans, coordinate with the home health team, educate patients and families, and document progress with accurate clinical notes. Texas licensure and a valid driving record are required.

Qualifications

• Licensed Occupational Therapist in Texas.
• Home health experience preferred.
• Knowledge of home health regulations and compliance.

Responsibilities

• Facilitate rehabilitation of patients with emotional, physical, or mental disabilities.
• Develop and implement occupational therapy care plans.
• Coordinate care with home healthcare team.
• Evaluate therapy needs and monitor and report patient progress and outcomes.
• Educate patients and family members.
• Prepare timely, complete, and accurate clinical notes.
• Communicate effectively with patients, caregivers, healthcare providers, office support, and other care team members.
