Job summary

Transition Home Healthcare is seeking a dedicated Physical Therapist (PT) to provide skilled home health services in Houston, TX and surrounding areas. The role offers PRN and FT shifts, with a focus on delivering high-quality patient care within the home setting.

Responsibilities include implementing care plans, coordinating with the home health team, educating patients and families, and maintaining accurate clinical notes.

Qualifications

• Licensed Physical Therapist in Texas.
• Home health experience preferred.
• Knowledge of home health regulations and healthcare compliance.
• Experience with HCHB EMR preferred.
• Able to handle physical workload and have good driving record.
• Excellent communication and people skills.

Responsibilities

• Facilitate the rehabilitation of patients with emotional, physical or mental disabilities.
• Implement physical therapy care plans.
• Coordinate care with the home healthcare team.
• Educate patients and family members about health conditions.
• Prepare timely, complete, and accurate clinical notes.
• Communicate effectively with patients, caregivers, healthcare providers, office support, and other care team members.
