Job summary

Transition Home Healthcare in Houston, TX is seeking a dedicated Certified Occupational Therapist Assistant (COTA). The role covers full-time and PRN shifts within our home health service areas in the Southwest and Southeast.

You will implement care plans, educate families, and coordinate with the care team to ensure quality patient outcomes. Ideal candidates hold a Texas COTA license, have home health experience, and can manage clinical documentation accurately.

Qualifications

• Certified Occupational Therapist Assistant licensed in Texas.
• Home health care experience preferred.
• Knowledge of home health regulations and healthcare compliance.

Responsibilities

• Facilitate rehabilitation of patients with emotional, physical or mental disabilities.
• Implement occupational therapy care plans.
• Coordinate care with the entire care team.
• Educate patients and family members.
• Prepare timely, complete and accurate clinical notes.
• Communicate effectively with patients, caregivers and providers.

Responsibilities:

• Facilitation of rehabilitation of patients with emotional, physical or mental disabilities
• Implementation of occupational therapy care plans
• Coordination of care with entire care team
• Educating patients and family members
• Preparing timely, complete and accurate clinical notes
• Communicating effectively with patients, caregivers, healthcare providers, office support and other care team members

Qualifications:
