PACS Group is seeking a compassionate Occupational Therapist (OT) to join the Santa Cruz Post Acute rehabilitation team in California. The role offers full-time, part-time, or PRN schedules, starting at $50 per hour with DOE plus benefits.

You will evaluate residents, develop individualized treatment plans, and collaborate with an interdisciplinary team to deliver high-quality, evidence-based care while educating patients and families on home-care strategies.

Qualifications

• Licensed Occupational Therapist in the state of California.
• Graduate of an accredited Occupational Therapy program.
• Strong interpersonal, communication, and documentation skills.

Responsabilités

• Evaluate, plan, and implement therapy programs to help residents regain functional independence.
• Provide direct therapeutic interventions to improve ADLs, mobility, strength, and independence.
• Document evaluations, treatment notes, progress reports, and discharge summaries.
• Collaborate with nursing, therapy staff, and physicians for coordinated care.
• Educate residents and families on safe techniques and home care strategies.
• Participate in care conferences and interdisciplinary planning.
• Maintain compliance with state, federal, and professional guidelines.
