Nassau Rehabilitation and Nursing Center in Hempstead, NY is seeking a licensed Occupational Therapist (OT) to join our interdisciplinary rehabilitation team on a full-time basis. The role involves assessments, program development, and patient-centered interventions to improve daily living and functional skills.

The ideal candidate holds a NY state OT license, NBCOT certification, and 2+ years of experience (preferably in nursing home settings).

Qualifications

• Bachelor’s or Master’s degree in Occupational Therapy from an accredited program.
• Current New York State Occupational Therapist license.
• NBCOT certification.
• Minimum of 2 years of experience as an Occupational Therapist, preferably in nursing homes.

Responsibilities

• Assess patients’ physical, cognitive, and emotional abilities to identify areas for improvement.
• Develop and implement individualized treatment plans tailored to patient needs.
• Provide therapeutic activities and interventions to improve daily living and functional skills.
• Educate patients and families on coping strategies, adaptive techniques, and assistive devices.
• Collaborate with physicians, nurses, and other rehabilitation professionals to ensure comprehensive care.
• Monitor patient progress and modify treatment plans as needed.
• Maintain accurate documentation of evaluations, treatment plans, and progress notes.
• Advocate for patient needs across care settings.
• Assist patients with transitions back to daily activities.
• Conduct home or workplace assessments and recommend modifications when appropriate.
• Stay current with advancements and best practices in occupational therapy.

Candidate Requirements

• Bachelor’s or Master’s degree in Occupational Therapy from an accredited program
• Current New York State Occupational Therapist license
• Certification from the National Board for Certification in Occupational Therapy (NBCOT)
• Minimum of 2 years of experience as an Occupational Therapist
• Nursing home or skilled nursing experience preferred
• Strong knowledge of human anatomy, physiology, and psychology
• Proficiency with electronic health records (EHR), including Net Health and Sigma (preferred)
• Excellent communication, problem-solving, and critical-thinking skills
• Ability to work independently and collaboratively within a multidisciplinary team
