Job summary

Park Avenue Extended Care in Long Beach, NY seeks an Occupational Therapist (OT) to join our rehabilitation team. This full-time position offers a daytime schedule from 8:30 AM to 4:30 PM, with a pay range of $42-$44 per hour, commensurate with experience.

• Graduate of an accredited Occupational Therapy program.

• Deliver evidence-based interventions that promote independence in daily living.
• Monitor progress, document outcomes, and adjust treatment plans as needed.
• Collaborate with a skilled multidisciplinary team to ensure residents receive the highest quality care.
• Support residents recovering from illness, injury, or disability with personalized rehabilitation services.
• Maintain accurate documentation and safeguard resident confidentiality.
• Create a positive, respectful, and compassionate environment for residents and families.
