Job summary

Achieve Rehab & Nursing Facility in Liberty, NY is hiring a Full-Time Occupational Therapist to join our in-house rehabilitation team.

You will evaluate residents, develop OT treatment plans, address ADLs and mobility, and collaborate with physicians and nursing staff to maximize outcomes. We value resident-centered care and provide support for professional growth.

Qualifications

• Current New York OT license or eligibility.
• Graduate of an accredited Occupational Therapy program.
• New graduates are encouraged to apply.
• SNF/Long-Term Care experience preferred but not required.
• Experience with Net Health and PCC preferred.
• Strong communication, clinical reasoning, and organizational skills.
• Passion for resident-centered care.

Responsibilities

• Evaluate residents and develop individualized OT treatment plans.
• Provide skilled OT services to short-term and long-term care residents.
• Address ADLs, upper extremity function, cognition, adaptive equipment needs, and mobility.
• Monitor progress and adjust plans based on outcomes.
• Educate residents, families, and caregivers on interventions and safety.
• Document accurately using Net Health and PCC.
• Collaborate with physicians and interdisciplinary team to maximize outcomes.
• Maintain compliance with all regulations.

Achieve Rehab & Nursing Facility is seeking a compassionate and dedicated Full-Time Occupational Therapist (OT) to join our in-house rehabilitation team in Liberty, New York.

Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans.
• Provide skilled occupational therapy services to short-term rehabilitation and long-term care residents.
• Address activities of daily living (ADLs), upper extremity function, cognition, adaptive equipment needs, and functional mobility.
• Monitor resident progress and modify treatment plans based on clinical outcomes.
• Educate residents, families, and caregivers on therapeutic interventions, safety techniques, and adaptive strategies.
• Complete accurate and timely documentation using Net Health and PointClickCare (PCC).
• Collaborate with physicians, nursing staff, and the interdisciplinary care team to maximize resident outcomes.
• Maintain compliance with all federal, state, and facility regulations.

Qualifications

• Current New York Occupational Therapist (OT) license or eligibility for licensure.
• Graduate of an accredited Occupational Therapy program.
• New graduates are encouraged to apply.
• Skilled Nursing Facility (SNF) or Long-Term Care experience is preferred but not required.
• Experience with Net Health and PointClickCare (PCC) is preferred.
