Job summary

Santa Rosa Post Acute is seeking a compassionate Occupational Therapist to join our rehabilitation team. The OT will evaluate residents, develop treatment plans, and help patients regain independence, with a focus on ADLs, mobility, cognition, and functional outcomes.

We offer a sign-on bonus of $2,000 for full-time new hires, 65–75 per hour, and both full-time and part-time schedules. California OT license required; experience in geriatric post-acute settings is preferred.

Qualifications

  • Current California OT license in good standing.
  • Graduate of an accredited Occupational Therapy program.
  • Experience in geriatric/post‑acute rehab preferred.
  • New graduates encouraged to apply with strong clinical rotations.
  • Experience with EMR systems such as Casamba, Net Health or PointClickCare is a plus.
  • Strong communication, organizational and teamwork skills.
  • Passion for helping patients regain independence.

Responsibilities

  • Evaluate residents and develop individualized OT treatment plans.
  • Provide skilled OT interventions per physician orders and care plans.
  • Improve ADLs, self-care, mobility, cognition and functional independence.
  • Collaborate with PT/SLP, nursing, physicians and families to coordinate care.
  • Complete timely, accurate documentation per regulatory and facility requirements.
  • Participate in interdisciplinary team meetings and discharge planning.
  • Educate patients and caregivers on therapeutic techniques and home programs.
  • Maintain compliance with standards of practice.

Occupational Therapist (OT) Santa Rosa Post Acute

Santa Rosa Post Acute is seeking a compassionate, motivated, and patient-centered Occupational Therapist (OT) to join our growing rehabilitation team. Sign‑On Bonus

Full‑time new hires are eligible for a 2,000 dollar sign‑on bonus, paid in two installments: 1,000 (50%) after completion of 90 days of continuous employment; 1,000 (50%) after completion of 180 days of continuous employment. Bonus payments are contingent upon remaining actively employed and in good standing at the time each payment is due.

Responsibilities
  • Evaluate residents and develop individualized occupational therapy treatment plans
  • Provide skilled occupational therapy interventions in accordance with physician orders and established care plans
  • Help patients improve performance in activities of daily living (ADLs), self‑care tasks, mobility, cognition, and functional independence
  • Collaborate with Physical Therapists, Speech Therapists, nursing staff, physicians, and families to coordinate care
  • Complete timely and accurate documentation in compliance with regulatory and facility requirements
  • Participate in interdisciplinary team meetings, care conferences, and discharge planning
  • Educate patients, families, and caregivers regarding therapeutic techniques and home programs
  • Maintain compliance with all federal, state, and professional standards of practice
Qualifications
  • Current California Occupational Therapist (OT) license in good standing
  • Graduate of an accredited Occupational Therapy program
  • Skilled nursing, post‑acute, rehabilitation, or geriatric experience preferred
  • New graduates with strong clinical rotations are encouraged to apply
  • Experience with Casamba, Net Health, PointClickCare, or similar EMR systems is a plus
  • Strong communication, organizational, and teamwork skills
