Blue Oak Post-Acute in Santa Rosa, CA is seeking a Physical Therapist to provide high-quality rehabilitative care to residents. You will evaluate residents, develop individualized plans, and implement evidence-based interventions to improve mobility and independence.

You may supervise PT assistants, document progress, and collaborate with physicians and interdisciplinary staff. New graduates welcome; experience in skilled nursing or rehab preferred.

Qualificações

• Active California Physical Therapist license in good standing.
• Bachelor’s, Master’s, or Doctorate degree in Physical Therapy.
• New graduates are welcome to apply; experience in skilled nursing or rehabilitation settings preferred.
• Knowledge of PDPM, Medicare, and skilled nursing facility regulations.
• Strong leadership, communication, and organizational skills.
• Commitment to resident-centered, high-quality care.
