Job summary

Camino Ridge Post Acute in Mountain View, CA is seeking an Occupational Therapist (OT) to assess residents and design personalized therapy plans. The role focuses on maximizing independence and safety through evidence-based interventions in a collaborative rehab team.

Qualifications

• Active California OT license in good standing.
• Bachelor’s or Master’s degree in occupational therapy.
• New graduates welcome; experience in skilled nursing or rehab settings preferred.
• Knowledge of PDPM, Medicare and SNF regulations.

Responsibilities

• Evaluate residents and develop individualized OT treatment plans.
• Implement evidence-based interventions to support independence.
• Document progress accurately and timely per regulatory requirements.
• Collaborate with interdisciplinary teams to optimize resident care.
• Participate in care conferences and family meetings as applicable.
• Mentor OT assistants and support staff as applicable.
