Qualifications

Responsibilities

• Provide occupational therapy services to patients in SNF.
• Assess patient function and develop individualized treatment plans.
• Focus on ADLs, mobility and independence to improve quality of life.
• Collaborate with an interdisciplinary team to ensure best outcomes.
• Educate patients and families on adaptive techniques and equipment.
• Maintain accurate and timely documentation.
