Crest Haven is seeking an Occupational Therapist (OT) to join their team in Cape May Courthouse, NJ. You will assess residents, develop individualized care plans, and support independence in ADLs using adaptive equipment and environmental modifications.

Responsabilités

• Assess and evaluate residents' functional abilities focusing on activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
• Develop tailored intervention plans to improve residents' independence in daily activities.
• Ensure accurate and consistent collection of Section GG data for Annual, Quarterly, Falls, Significant Change assessments, BIMS, PHQ 2-9, and other clinical data as needed.
• Administer OT interventions, including ADL training, adaptive equipment provision, and environmental modifications.
• Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress. Conduct remote check-in through remote therapeutic monitoring (RTM) with discharged patients to home or next level of care.
• Maintain detailed and up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, and care updates in EMRs (Net Health/PCC).
• Collaborate with other rehab staff, such as Physical Therapists and Speech-Language Pathologists, for holistic resident care.

• Interdisciplinary Collaboration: Collaborate with other rehab staff, such as Physical Therapists and Speech-Language Pathologists, for holistic resident care.
