Crest Haven Nursing and Rehab in Cape May Courthouse, NJ is seeking a dedicated Physical Therapist to join our team. The role focuses on comprehensive assessments, tailored treatment plans, and contributing to interdisciplinary care to improve residents’ mobility and comfort.

Responsibilities include conducting mobility-focused therapies, collecting Section GG data, educating residents and families, and maintaining meticulous documentation in EMRs.

• Assessment and Evaluation: Conduct comprehensive physical assessments to determine residents' impairments and functional limitations.
• Individualized Care Plans: Develop treatment plans with goals for mobility and pain reduction.
• Section GG Data Collection: Collect data for Annual/Quarterly/Falls assessments.
• Treatment: Administer physical therapy interventions to enhance function.
• Education: Educate residents and staff on safe mobility techniques and exercises.
• Documentation: Maintain up-to-date documentation in EMRs.
• Interdisciplinary Collaboration: Work with rehab team including OT and SLP.

Responsabilities:

• Assessment and Evaluation: Conduct comprehensive physical assessments to determine residents' physical impairments and functional limitations, focusing on mobility, balance, and gait.
• Individualized Care Plans: Develop individualized treatment plans based on assessments, with clear goals for improving residents' mobility and reducing pain.
• Section GG Data Collection: Accurately and consistently collect Section GG data as required for Annual, Quarterly, Falls, and Significant Change assessments.
• Treatment: Administer physical therapy interventions, including exercises, manual techniques, and modalities, to enhance residents' physical function.
• Education: Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress. Conducts remote check-in through remote therapeutic monitoring(RTM) with discharged patients to home or next level of care.
• Documentation: Comfortable and knowledgeable with Point of Service( PoS) care delivery and documentation. Maintain detailed and up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, HMO/Manage care progress updates in EMRs( Net Health/PCC).
• Interdisciplinary Collaboration: Collaborate with the rehab team, including Occupational Therapists and Speech-Language Pathologists, to ensure comprehensive care for residents.
