Crest Haven Nursing and Rehab in Cape May Court House, NJ is seeking an Occupational Therapist to join their team.

This beautiful nursing home emphasizes leadership, a warm work environment, and excellence in resident care. You will assess residents' ADLs/IADLs, develop individualized care plans, collect Section GG data, provide OT interventions, educate families and staff, and collaborate with PTs and SLPs. Remote check-ins via RTM are also offered.

Qualifications

• Assessment and Evaluation: Perform comprehensive assessments of residents' functional abilities, with a focus on activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
• Individualized Care Plans: Develop tailored intervention plans to improve residents' independence in daily activities.
• Section GG Data Collection: Ensure accurate and consistent collection of Section GG data for Annual, Quarterly, Falls, Significant Change assessments, BIMS, PHQ 2-9, and other clinical data as needed.
• Treatment: Administer OT interventions, including ADL training, adaptive equipment provision, and environmental modifications.
• Education: Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress. Conducts remote check-in through remote therapeutic monitoring(RTM) with discharged patients to home or next level of care.
• Documentation: Comfortable and knowledgeable with Point of Service( PoS) care delivery and documentation. Maintain detailed and up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, HMO/Manage care progress updates in EMRs( Net Health/PCC).
• Interdisciplinary Collaboration: Collaborate with other rehab staff, including Physical Therapists and Speech-Language Pathologists, for holistic resident care.
