Job summary

Ultimate Therapy is hiring an Occupational Therapist (OT) – Full-Time to become part of the rehabilitation team at Autumn Lake at Nellas. Work directly with residents to strengthen everyday skills, improve functional abilities, and promote greater independence in a supportive skilled nursing and post-acute care environment.

You’ll evaluate residents, develop tailored treatment plans, address ADLs, balance, cognition, and coordinate with clinicians to ensure comprehensive care.

Qualifications

• Active West Virginia OT license required.
• Graduate of an accredited Occupational Therapy program.
• Strong assessment, communication, and organizational skills.
• New graduate OTs are encouraged to apply.

Responsibilities

• Evaluate residents and create individualized occupational therapy treatment plans
• Provide skilled interventions addressing activities of daily living, strength, coordination, balance, cognition, and functional performance
• Help residents develop strategies to improve independence with everyday tasks
• Monitor outcomes and modify treatment plans according to resident progress and needs
• Recommend adaptive equipment and provide education on safe, effective use
• Complete thorough and timely clinical documentation
• Coordinate care with COTAs, PT, SLP, nursing, and other healthcare professionals
