Job summary

Park Avenue Extended Care in Long Beach, NY is seeking a full-time Occupational Therapist to join our rehabilitation team. The OT will assess residents, develop individualized plans, and promote independence in daily living with evidence-based interventions.

Requirements include graduating from an accredited OT program and an active New York State OT license. Prior long-term care experience is preferred but not required. We offer a supportive team environment and comprehensive benefits.

Salary: $42-$44 per hour (commensurate with experience)

Key Responsibilities

• Conduct comprehensive evaluations and design individualized therapy plans tailored to residents’ needs and goals.

• Deliver evidence-based interventions that promote independence in daily living.

• Collaborate with a skilled multidisciplinary team to ensure residents receive the highest quality care.

• Support residents recovering from illness, injury, or disability with personalized rehabilitation services.

Qualifications

• Graduate of an accredited Occupational Therapy program.

• Active New York State Occupational Therapist license.

• Prior experience in long-term care or skilled nursing facilities preferred, but not required.
