Job summary

Crown Park Rehabilitation and Nursing Center in Cortland, NY is seeking an Occupational Therapist to evaluate and develop treatment plans for residents, implement plans to improve independence, and collaborate with care teams across the facility.

New graduates are welcome. Applicants must hold a NY State OT license. The position offers competitive weekly pay and a comprehensive benefits package including health insurance, tuition reimbursement, and PTO.

Qualifications

• Graduate of an accredited Occupational Therapy Program.
• Current New York State Occupational Therapist License.
• Experience is a plus, though not required.

Responsibilities

• Evaluate and develop treatment plans for residents based on their medical diagnoses.
• Implement treatment plans to improve residents' goals toward functional independence.
• Assess and document residents' progress in detailed re-certifications, progress reports, and daily notes.
• Maintain resident confidence by ensuring the confidentiality of all information.
• Maintain documentation of resident care services by auditing resident and department records.
• Assume responsibility for clinically supervising occupational therapy assistants.
• Participate in care plan meetings and contribute to related documentation.
• Plan, develop, and organize OT-related reports and data tracking as directed by the Rehab Director.
• Assist in departmental projects as needed.
