Job summary

The Grand Healthcare is seeking an Occupational Therapist to evaluate, develop, and implement treatment plans aimed at helping residents achieve functional independence. You will work with a compassionate team to support daily living activities.

Qualifications

• Graduate of an accredited Occupational Therapy Program.
• Current New York State OT License.
• Experience is a plus, though not required.

Responsibilities

• Evaluate and develop treatment plans for residents based on their medical diagnoses.
• Implement treatment plans to improve residents' goals toward functional independence.
• Assess and document residents' progress in re-certifications, progress reports, and daily notes.
• Maintain resident confidentiality of all information.
• Maintain documentation of resident care services by auditing records.
• Assume responsibility for clinically supervising occupational therapy assistants.
• Participate in care plan meetings and contribute to related documentation.
• Plan, develop, and organize OT-related reports and data tracking as directed by Rehab Director.
• Assist in departmental projects as needed.

As an Occupational Therapist, you will play a key role in evaluating, developing, and implementing treatment plans for residents, working to help them achieve functional independence.

Hourly Rate: $42
