Job summary

Reliant Rehabilitation seeks a licensed Occupational Therapist to provide therapy services in Assisted Living, Independent Living, or Memory Care settings. This PRN/day shift role requires screening, evaluation, treatment planning, and discharge planning while collaborating with the rehab team to deliver patient-centered care.

Responsibilities include supervising COTAs and Rehab Techs, ensuring compliant documentation, and adhering to OBRA and company policies.

Qualifications

• Competencies: strong professional conduct and adherence to regulatory guidelines.

Responsibilities

• Screen residents as appropriate; evaluate and treat patients who may benefit from therapy.
• Provide OT services including screening, evaluation, and development of initial and ongoing treatment plans; discharge planning.
• Supervise Certified Occupational Therapy Assistants (COTA) and Rehab Techs; provide consultation and clinical supervision.
• Ensure accurate and timely billing and documentation in the medical record; maintain patient-centered care in collaboration with the interdisciplinary team.
• Travel between communities during scheduled hours; work varying hours including weekends and holidays.
• Maintain infection control, state practice acts compliance, and patient rights/privacy.
