Job summary

WelbeHealth is seeking an Occupational Therapist to coordinate and provide restorative and rehabilitative OT services within our program, working with the PACE Physician, rehab staff, and IDT to maximize independence and safety in ADLs.

This role features lower patient volumes, long-term relationships with participants, collaboration with other therapists and staff, and a balanced schedule with no after-hours documentation.

Qualifications

• Bachelor’s degree in occupational therapy; master’s preferred.
• Valid California Occupational Therapy License, or proof of license eligibility.
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred

Responsibilities

• Conduct Occupational Therapy assessments in our facility and occasionally in the home to define participant care needs, preferences, and goals
• Actively participate in IDT meetings to develop participant care plans
• Deliver and document interventions as agreed upon in participant care plans
• Provide active and passive exercise, muscle re-education, gait, and functional training of ADLs
• Work with Therapy Assistants, providing training, instruction, and guidance needed to implement Functional Maintenance Programs (FMP)
