Job summary

Omega Home Health Care Services, Inc. is seeking a compassionate Occupational Therapist to join our home health team on a per diem basis. You will provide rehabilitative services in patients' homes to help them regain independence and improve quality of life, adhering to physician orders and company protocols.

Candidates must be licensed in California and have at least 1 year of home health experience. A valid driver's license and reliable transportation are required.

Qualifications

• Current and valid OT license in California.
• Graduate of an accredited OT program.
• Minimum of 1 year experience in a home health or related setting preferred.
• Valid driver’s license and reliable transportation.
• Strong interpersonal, communication, and organizational skills.
• Proficiency with electronic medical records (EMR) systems is a plus.

Responsibilities

• Conduct comprehensive patient evaluations and develop individualized treatment plans.
• Deliver occupational therapy services in patients’ homes in accordance with physician orders and company protocols.
• Instruct patients and caregivers in use of adaptive equipment and techniques to improve daily living skills.
• Monitor patient progress and modify treatment plans as needed.
• Collaborate with multidisciplinary team including nurses, physical therapists, and social workers.
• Maintain accurate and timely documentation in compliance with California state and federal regulations.
• Ensure patient safety and adherence to infection control procedures in home environments.

This position is paid by the visit. Actual pay rates vary depending on factors including type of visit, type of experience, location, and years of experience.
