Location: Stockton

Scheduled: Mon-Friday

Responsibilities

• Provide occupational therapy treatments as ordered by the physician
• Assist in developing and updating the Plan of Care (POC)
• Evaluate patient functional levels and progress
• Observe, record, and report patient condition changes
• Teach and guide family members and Lorian staff on therapy programs
• Supervise other healthcare personnel when appropriate
• Assess the home environment and recommend modifications
• Complete and submit accurate, timely clinical documentation per Lorian policies

Qualifications

• Registered by the Occupational Therapist Association and licensed in the state of practice if the state has registered practices.
• Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.
• Possesses and maintains good physical stamina and mental health.
• Presents a pre‑employment physician’s health clearance, including negative TB skin tests and/or CXR and other tests as required by specific state statute.
• Has the ability to read and follow written instructions and document care given.
• Is self‑directing and has the ability to work with little direct supervision.
• Possesses and maintains current CPR certification.
• Is flexible and cooperative in fulfilling the role obligations.
• Demonstrates tact, patience, and good personal hygiene.
• Is a licensed driver and has an automobile that is insured in accordance with Lorian Health (LORIAN) requirements.
• Must be able to document all client‑related activity practices.

This position is based on a Pay per visit compensation model.

Pay per visit ranges from $50 to $140 visit commensurate with experience and depending on type of visit.
