Job summary

Meadows Post Acute is seeking a licensed Occupational Therapist to join our skilled nursing facility team in California. The OT will evaluate and treat residents with physical, cognitive, and psychosocial impairments to help them achieve independence and improve quality of life.

Responsibilities include assessments, individualized treatment plans, therapy interventions, progress monitoring, and interdisciplinary coordination.

Qualifications

  • Current license to practice as an Occupational Therapist in California.
  • Minimum of 2 years of experience in a healthcare setting, preferably skilled nursing.
  • Knowledge of current occupational therapy practice standards and regulations.
  • Proficient in using electronic medical records and other healthcare software.

  • Evaluate residents' physical, cognitive, and psychosocial abilities and functional limitations using standardized assessments.
  • Develop and implement individualized treatment plans based on assessment results and physician orders.
  • Provide skilled occupational therapy interventions, including therapeutic exercise, ADL training, cognitive retraining, and environmental modifications.
  • Monitor and document resident progress and adjust treatment plans as necessary.
  • Communicate with interdisciplinary team members to coordinate care and ensure optimal outcomes for residents.
  • Educate residents and family members on occupational therapy goals and home exercise programs.
  • Maintain accurate and timely documentation of occupational therapy services provided.
  • Participate in team meetings, in-service education, and quality improvement initiatives.
  • Maintain current knowledge of occupational therapy practice and relevant regulations.

  • Participate in team meetings, in-service education, and quality improvement initiatives
  • Maintain current knowledge of occupational therapy practice and relevant regulations
