Job summary

Lone Tree Post Acute in Antioch, CA is seeking a licensed Occupational Therapist (OT) to join our in-house rehabilitation team.

Evaluate residents and develop individualized treatment plans while delivering skilled OT services to promote independence and quality of life. You will collaborate with physicians, nursing, and the interdisciplinary team, supervise COTAs and students, and participate in discharge planning and family education.

Qualifications

• Current, active California Occupational Therapist (OT) license
• Strong clinical reasoning and problem-solving skills
• Excellent communication and interpersonal abilities
• Commitment to compassionate, resident-centered care
• Ability to work effectively as part of a multidisciplinary team

Responsibilities

• Evaluate residents and develop individualized, evidence-based treatment plans
• Deliver skilled occupational therapy services that promote functional independence and quality of life
• Collaborate with physicians, nursing staff, and the interdisciplinary care team
• Supervise and mentor COTAs and therapy students
• Participate in care conferences and discharge planning
• Educate residents, families, and caregivers on therapeutic techniques and home safety
• Maintain accurate, timely documentation and ensure regulatory compliance
