Job summary

Lone Tree Post Acute in Antioch, CA is seeking a compassionate Occupational Therapist (OT) to join our in-house rehabilitation team. You will evaluate residents, develop evidence-based treatment plans, and support residents to regain independence and maximize function.

Responsibilities

• Supervise and mentor COTAs and therapy students
• Participate in care planning meetings and interdisciplinary collaboration
• Maintain accurate clinical documentation and ensure regulatory compliance
