Job summary

Lone Tree Post Acute in Antioch, CA is seeking a licensed Occupational Therapist to join our in-house rehabilitation team. You will evaluate residents and develop individualized, evidence-based treatment plans to promote functional independence.

Collaborate with physicians, nurses, and COTAs to achieve optimal outcomes, supervise students, participate in care conferences, and educate residents and families on home safety and therapeutic techniques.

Qualifications

• Current, active California Occupational Therapist license required.
• Strong clinical reasoning and problem-solving abilities.
• Excellent communication and interpersonal skills.

Responsibilities

• Evaluate residents and develop individualized, evidence-based treatment plans.
• Deliver skilled occupational therapy services to promote functional independence.
• Collaborate with physicians, nursing staff, and the care team for optimal outcomes.
• Supervise COTAs and therapy students.
• Participate in care conferences and discharge planning.
• Educate residents and families on home safety and therapeutic techniques.
