Job summary

Lone Tree Post Acute in Antioch, CA is seeking an Occupational Therapist (OT) to join our in-house rehabilitation team. You will evaluate residents and develop individualized, evidence-based treatment plans to promote functional independence and quality of life.

Deliver skilled OT services, collaborate with physicians and nursing staff, supervise COTAs and therapy students, participate in care conferences and discharge planning, and educate residents, families on home safety.

Qualifications

• Current, active California Occupational Therapist (OT) license.

Responsibilities

• Evaluate residents and develop individualized, evidence-based treatment plans.
• Deliver skilled occupational therapy services that promote functional independence and improved quality of life.
• Collaborate with physicians, nursing staff, and the interdisciplinary care team.
• Supervise and mentor Certified Occupational Therapy Assistants (COTAs) and therapy students.
• Participate in care conferences and discharge planning.
• Educate residents, families, and caregivers on therapeutic techniques and home safety.
• Maintain accurate, timely documentation and ensure regulatory compliance.
