Job summary

Evolve Therapy Services, LLC is hiring an Occupational Therapist for an in-house Skilled Nursing Facility in Olivia, MN. You will plan, organize, develop, and direct Occupational Therapy services to ensure the highest quality resident care in compliance with standards.

Qualifications

• Degree in Occupational Therapy is required.
• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Conduct regular screening of residents to determine best intervention or treatment.
• Evaluate residents to inform and develop treatment plans; set short- and long-term goals.
• Interpret and communicate evaluation findings and recommendations to residents and families.
• Establish resident therapy schedules with other team members.
• Conduct therapy per treatment plan and ensure documentation and billing.
• Perform other duties as assigned to effectively treat patients.
• Oversee COTA as needed and ensure proper reimbursement practices.
