Job summary

Evolve Therapy Services, LLC in Olivia, MN is seeking an Occupational Therapist to lead in-house Skilled Nursing Facility services. You will assess residents, develop treatment plans, and collaborate with an interdisciplinary team to maximize resident function.

Qualifications

• Must hold an active Occupational Therapy license in practicing state(s)
• Ongoing continuing education to maintain licensure and certification

Responsibilities

• Conduct regular screening of residents to determine best intervention
• Evaluate residents to inform and develop treatment plans with goals
• Interpret and communicate evaluation findings and recommendations to residents and families
• Establish individual resident therapy schedules with the team
• Conduct therapy per treatment plan and ensure proper documentation and billing
• Perform all duties to effectively treat patients
• Oversee COTA and ensure documentation and treatments meet reimbursement practices
