Job summary

ABL Health Care LLC is seeking a compassionate Occupational Therapist (OT) for full-time Home Health in Alameda. The OT will evaluate patients, design and implement care plans, and track progress in the home setting, working with the Clinical Director and team.

Required: CA OT license, 1 year of Home Health or Hospice experience, CA driver’s license and ability to travel within 15–25 miles. We offer competitive benefits including medical, dental, vision, 401(k), and PTO.

Qualifications

• Current OT license to practice in California
• Experience in Home Health or Hospice care (1 year minimum)
• Valid California driver’s license and ability to travel within 15–25 miles

Responsibilities

• Provide, coordinate and oversee skilled intermittent care in the patient’s home.
• Regularly evaluate and re-evaluate patient status.
• Administer treatments and monitor patient conditions; report changes to the physician/case manager.
• Initiate, develop, implement and revise the patient’s plan of care (POC).
• Educate patient, family and caregivers about the condition and home care needs.
• Maintain accurate clinical records and submit documentation per policy.
