Job summary

ABL Health Care in San Leandro is seeking an experienced Occupational Therapist (OT) for full-time Home Health. You will evaluate patients, craft and implement care plans, and monitor progress while working with the Clinical Director and field staff.

Qualifications

Current OT License to practice in California is required.
Experience in Home Health or Hospice care is required (1 year).
Current California driver's license is required.
Ability to travel within a 15-25 mile radius on a regular basis.

Responsibilities

Provide, coordinate and oversee skilled intermittent care to patients in the home.
Regularly evaluate and re-evaluate patient status.
Administer treatments, monitor patient conditions and report changes to the physician/case manager.
Initiate, develop, implement and revise the patient's plan of care (POC).
Address problems identified in the POC.
Educate patient/family/caregiver about the condition and care needs.
Communicate with other disciplines involved in the case as warranted.
Maintain accurate clinical records and submit documentation using a company-provided device.
Comply with all company policies and procedures to ensure high standards of care.
