Job summary

ABL Health Care is hiring a Full-Time Occupational Therapist (OT) for Home Health in Alameda. You will evaluate patients, develop care plans, and monitor progress in home settings, working with the Clinical Director and staff in the field.

We seek an organized, compassionate OT licensed in California, capable of remote support and field visits within the specified radius, with travel as part of the role. Competitive benefits and equipment are provided.

Qualifications

• Current California OT license required.
• Experience in Home Health or Hospice care (1 year required).
• Valid California driver’s license.
• Willingness to travel within a 15–25 mile radius.

Responsibilities

• Provide and oversee skilled intermittent care in patients' homes.
• Regularly evaluates and re-evaluates patient status.
• Administer treatments, monitor patient conditions, and report changes to the care team.
• Initiates, develops, implements and revises the patient's plan of care (POC).
• Maintains accurate clinical records and complies with company policies.
