Job summary

ABL Health Care LLC in Berkeley is seeking a full-time Occupational Therapist (OT) to join our Home Health team. You will evaluate patients, create and implement care plans, and track progress, collaborating with the Clinical Director and staff both in the field and remotely.

The role requires a California OT license, at least 1 year in home health or hospice, and a valid CA driver’s license. Travel within a 15-25 mile radius is expected and you will enjoy support from back-office clinicians and

Qualifications

• CA OT license required.
• Experience in Home Health or Hospice care preferred (1 year).
• Needs valid CA driver license and ability to travel within a regular radius.

Responsibilities

• Provide, coordinate and oversee skilled intermittent care in patients' homes.
• Evaluate and re-evaluate patient status and adjust care plans as needed.
• Administer treatments, monitor conditions, and report changes to supervising clinicians.
• Develop and revise the patient's plan of care (POC) and educate patient/family.
