Job summary

ABL Health Care LLC in Half Moon Bay is seeking a Full-Time Occupational Therapist (OT) to join our Home Health team. You will evaluate patients, develop and carry out care plans, and monitor progress in patients' homes, working with our clinical staff to ensure high-quality, compassionate care.

The ideal candidate holds a CA OT license, has 1 year in Home Health or Hospice, a valid CA driver's license, and can travel within a 15–25 mile radius.

Qualifications

• Current OT License to practice in California is required.
• Experience in Home Health or Hospice care is required (1 year).
• Current CA Driver's License is required.
• Able to travel within a 15-25 mile radius on a regular basis.

Responsibilities

• Provide, coordinate and oversee skilled intermittent care to patients in the home.
• Regularly evaluate and re-evaluate patient status.
• Administer treatments, monitor conditions, and report changes to the appropriate team.
