Job summary

Pathways Healthcare in Concord, MA is seeking a professional Occupational Therapist to join our full-time team. The role involves conducting comprehensive patient assessments, developing individualized treatment plans, training patients in daily activities and prosthetic use, and coordinating discharge planning.

You will supervise occupational therapy assistants and home health aides, educate families, maintain records, and participate in team meetings to ensure high-quality home-based care for

Qualifications

• Hold a degree from an occupational therapy curriculum accredited jointly by the AMA Committee and AOTA and be registered with the National Registration Examination of the AOTA.
• A minimum of two years of clinical experience as an Occupational Therapist is preferred.
• Demonstrate excellent verbal and written communication skills along with strong organizational abilities to interact effectively with patients and the healthcare team.
• Maintain a current BLS CPR certification.
• Possess a valid driver’s license and own a reliable, insured vehicle.

Responsibilities

• Conduct comprehensive patient assessments, including functional status, home environment, and equipment needs.
• Evaluate work capacity and provide vocational training referrals when necessary.
• Develop and implement individualized treatment plans with achievable goals.
• Train patients in daily activities, prosthetics, orthotics, and adaptive equipment use.
• Maintain accurate records and report patient condition changes promptly.
• Educate families and caregivers on treatment plans.
• Supervise occupational therapy assistants and home health aides.
• Participate in training, meetings, and case reviews.
• Refer patients to additional services as needed and coordinate discharge planning.

• Service Area: Devens, Shirley, Ayer, Dunstable, Groton, Pepperell, Tyngsboro, Townsend, Fitchburg, Leominster, Lunenburg areas.

• Schedule: Monday-Friday

• Salary Range: $80,000 - $90,000
